Emergencies are taken promptly. Your appointment   To ensure your office visit goes smoothly you should bring the following information: The name of your primary care physician. Your insurance information, including the insurance card and any insurance forms. If you participate with an HMO, please bring a referral form from your primary care physician. When you arrive at our office you will be greeted by the registration personnel and any additional information that may be needed will be taken.
