  What you'll need for your appointment

Our new office, located just off Route 37, has easy road access and ample parking. We are also close to our affiliate, Community Medical Center, a member of the Saint Barnabas Healthcare System. Our modern state-of-the-art office is equiped with a highly sophisticated computer system. This offers us the ability to streamline documentation of our medical charts, keeping them accurate up-to-the-minute. We are able to spend less time on forms and paperwork and more time providing you with the old fashioned one-on-one medical care you deserve.

Your appointment

To ensure your office visit goes smoothly you should bring the following information:

• The name of your primary care physician.
• Your insurance information, including the insurance card and any insurance forms.
• If you participate with an HMO, please bring a referral form from your primary care physician.

 
When you arrive at our office you will be greeted by the registration personnel and any additional information that may be needed will be taken. Any information given at this time is of course kept in the strictest confidence.
